  1. By telephone (see number above) to order by mail.
  2. Mail in refill Slip to Pharmacy Service
  3. Request at the front window for MAIL out

Co-Payment Options:
  1. Via Cashier's Window (Across from Pharmacy if you are at the Ambulatory Care Center in Honolulu).
  2. Via monthly bill to be sent to your mailing address.
