How to request VA records by Fax or Mail:

INDIVIDUAL REQUEST: To request your medical records please print the VA Form 10-5345a , complete the requested information, sign, and date the form.

THIRD PARTY REQUEST: To request your medical records be sent to a Third party i.e.: Doctors office, Insurance Company, etc. please print the VA Form 10-5345 , complete the requested information, sign, and date the form.  

*Forms must contain an original signature, e-mailed forms cannot be accepted. Request for records are processed within (20) business days of receiving your request.

How to request VA records in person:

SPARK M. MATSUNAGA BUILDING:

You have two options to request your records in person at the Spark Matsunaga building. You can do so by visiting the KIOSKs located throughout the building or You can visit the Check in desk on the 2nd floor.

E-WING:

Visit the KIOSK located in the lobby. If it is something needed right away, please see the Security Officer and he will call a representative.

FEES:

There is no cost to send copies to another health care provider. If copies are for a patient’s personal use, photocopying fees may be assessed.

PATIENT ADVOCATE OFFICE: If you have questions, concerns or positive feedback regarding service you have received while in our care please reach out to our Patient Advocate Office. You will find the Patient Advocate Office located in the Spark M. Matsunaga Building on the first floor, behind the security desk.
